Chameleon’s Journey Greenville Spring Day Camp Registration opens Monday, March 2nd!!


Our Greenville Spring Day Camp Registration opens on Monday, March 2nd and we want to pass along a few details to make your process as seamless as possible!  Below, we cover some of the specifics as well as a few commonly asked questions.  Don’t hesitate to contact us at 704.335.4300 with any specific concerns!


Q:  How do I register?


A:  On Monday, March 2nd, you will be able to RSVP for camp via the CJ website under “events.”  You will receive a confirmation e-mail AND an additional e-mail with a link for FormDr. which contains the actual camp application.  This application must be fully completed and submitted for EACH camper to be considered.  Please check your spam/clutter folder as some servers divert the FormDr. email link.



Q:  This seems complicated; why the extra step?


A:  The protection of your personal information is a top priority for VIA Health Partners.  FormDr. is HIPAA compliant and prevents spamware from intercepting or “hacking” personal data.  It also allows for electronic signatures to cut out an extra step in the process.  If you have any issues or don’t see the link, just give our office a call at the above number, and we can easily resend what you need.



Q:  Do I really need to complete a new application for each of my campers?


A:  Yes, we legally need consent for each camper.  It will only take about 10 minutes to complete the form, and you will need allergy information, health insurance data, medication list (if any) and any specific dietary needs.



Q:  Who can complete the application form?


A:  The application must be completed by a legal guardian or representative of the camper due to important consents related to camp and the use of our camp venue.



Q:  Can it be completed on my mobile device?


A:  Yes, the link for FormDr. can easily be opened on a laptop, desktop, tablet or smartphone device.



Q:  How will I know my camper has been accepted?


A:  You will receive an acceptance letter from our office including details for camp and a few other “extras” to consider.  Give us about a week after submitting your application to get that letter in the mail. 



Q:  How strict are the age guidelines?  My camper is 6 and will be 7 in December of 2025.


A:  Due to the legality of our insurance coverage, the nature of our clinical work and contracted agreement with camp venues, all campers must be at least 7 years old by the camp date.



Q:  When is the registration deadline?


A:  We are asking campers to please be registered up to the Friday before camp, to give us adequate time to be best prepared for your arrival.
